Rotator Cuff Exercises: Ease Shoulder Pain Gently

The rotator cuff is the group of small muscles that stabilize your shoulder, and it's a common source of pain and weakness after 50. Gentle rotator cuff exercises rebuild that stability and often ease pain, using light resistance and small, controlled movements — not heavy lifting. The best moves are external and internal rotations with a light band, scapular squeezes, and pendulum swings. Use very light resistance, stay below the pain threshold, and keep the elbow tucked for rotations. Do them most days, slowly. Avoid overhead lifting and anything sharply painful while the shoulder is irritated. If pain is severe, followed an injury, or you can't lift the arm, see a doctor — some rotator cuff problems need professional assessment.

What is the rotator cuff?

It's four small muscles that hold the shoulder joint centered and stable. With age they weaken and are prone to irritation and small tears, causing pain, weakness and trouble reaching overhead. Strengthening them gently restores stability and often reduces pain.

What to avoid while it hurts

Skip overhead lifting, heavy weights, and any movement that sharply increases the pain. Keep resistance very light — the rotator cuff responds to control, not load. Move slowly and stay below the pain threshold.

See a doctor if you can't raise the arm, the pain is severe, it followed a fall or injury, or there's significant weakness — these can signal a tear that needs assessment.
